What is IVF?

IVF or in vitro fertilisation involves the fertilisation of the woman's egg by the man's sperm outside of the body - usually in a glass dish. The fertilised egg or 'embryo' is then implanted into the woman's womb. This method of intervention has led to the use of the phrase 'test tube babies'. The procedure is used to help couples who are finding it impossible to conceive naturally and is most commonly used to help women with polycystic ovary syndrome or blocked fallopian tubes and men with poor sperm quality. It can be carried out using the couple's egg and sperm or with donor eggs and sperm. Each attempt (cycle) at IVF treatment is expensive, so there has to be a limit on how many times the treatment is provided to each couple by the NHS.

Who is eligible for IVF Treatment on the NHS?

In 2004 the National Institute for Clinical Excellence (NICE) provided guidelines for the NHS stating that 3 cycles of IVF should be provided for couples in which the woman is between 23 and 39 years of age. The couple should also have a diagnosis for their infertility or have been trying to conceive without success for at least 3 years.

In reality however, not all couples who meet these criteria are able to access IVF on the NHS. This is because different areas of the UK have different demands on their healthcare budgets. Some primary care trusts have had to stop providing IVF treatment altogether (for the time being at least) while most others have had to reduce the number of cycles of treatment given to women from three to just one or two. A Department of Health study in 2008 found that two thirds of Primary Care Trusts offered only one cycle of treatment per couple.

EVLT stands for Endovenous Laser Therapy. The veins can be seen as purplish, red or flesh coloured and are usually raised due to their enlargement. Because the legs have a lot of weight and gravity acting upon them, leg veins are the most common veins to become varicose.

For anyone who suffers with varicose veins, there can be a lot of distress caused with their unsightly appearance and the tendency to cause pain. Varicose veins are also potentially harmful because they can cause deep vein thrombosis (DVT), a blood clot that if dislodged, could then lodge in a vital organ, causing damage and even death. What is involved in EVLT?

The EVLT treatment is not suitable for everybody, and it is only suitable for certain varicose veins involving the long saphenous vein (LSV), which is behind the appearance of many varicose veins. Possibly 70% of people are eligible for EVLT. The standard surgical procedure involves making an incision into the groin area to cut the LSV and strip it away. The procedure is quite harsh. The EVLT procedure is recognised as being equally effective, but much gentler with a faster recovery time.

The patient is given a local anaesthetic prior to painlessly passing a wire up through the vein being treated, to the groin. A fine tube is threaded over the guiding wire, and the laser is fed along the tube. Its position is checked with ultrasound and when the surgeon is happy that he is at the right spot, he will inject further anaesthetic at the laser site, before commencing the laser. The laser will close the vein up from the inside; moving the laser slowly back along the vein whilst it is switched on seals the vein along its length. The laser is removed and the patient is bandaged.

Post procedurally, there is some discomfort for maybe two or three days, and there will be a need for bandages, then a support stocking. After seven days, the bandages are removed. Some minor bruising and visibility of the vein may remain but the body will break down the remnants of the vein and this will disappear.

EVLT is available privately. Typical costs are around £1700 for one leg, and £2000 for both legs, but this may vary. Some clinics are recognised through private health insurers to carry out the treatment. It is not currently available through the NHS.

Who is Eligible?

In order to qualify for free IVF treatments on the NHS, there are a certain number of criteria which must be met.
